Physically, the Nihilanth resembles a massive grayish-brown (pinkish under certain lighting conditions) fetus with abnormal proportions and a disproportionately large head perched atop a much smaller body. His tiny legs appear to be the remnants of an amputation, as the Nihilanth relies on a levitation device for mobility, a severe injury likely inflicted by the Combine. His head is so enormous that it must be supported by a thin cord or strap extending from the base of his spine to the back of his skull. Like other sapient species from Xen, he has three-fingered hands, multiple eyes (though distinctly different from the rest), and a third appendage protruding from the center of his chest. Beneath this third arm is a surgical scar running down his abdomen. His arms are unusually long, with massive hands and disproportionately large fingers.
